The Incident and Crime Report form is designed to aid Springfield residences in reporting information on certain crimes within the City of Springfield.

Download the form, so it can be filled-out on the computer and printed or printed and filled-out by hand. When your done filling-out the form, sign it, and mail to or bring it to the:

Your report will be logged. The form provides the foundation needed to begin investigating and resolving the situation. The SPD hopes to provide a variety of means for Springfield residents to contact the Department for support and help.

(If you are reporting a crime you witnessed but were not directly affected, please call 911 if it is an emergency. Or, send a e-mail the SPD and provide all the information you can about the situation. We will pass it on to the appropriated persons.)

In case of emergencies, dial 911. Give the SPD call takers as much information you can about your emergency. Most importantly, state you name, address, and the nature of your emergency.
