New applicants for a Taxi license need to meet the "Driver Requirements Checklist" before completing Taxi License application, a C.O.R.I. release form for a criminal history background check, and bring a Massachusetts Drivers License. There is a $25 non-refundable fee for the application process.

After the C.O.R.I. form returns from Boston, the application is reviewed by the Springfield Taxi Commission for licensing. Applicants will be notified by mail and can pick their license at the Records Division office at the Springfield Police Dept. at 130 Pearl St., Springfield, MA.

Driver Requirements Checklist
(Requirements must be met prior to filing your application.)

1. Applicants are required to be over 19 years old.
2. Applicants are required to have two (2) years driving experience.
3. Applicants must hold a valid Massachusetts license. Your license may not have been revoked, suspended, conditional, probationary, restricted or hold a non-renewable status.
4. Applicants are required to present an original Social Security card.
5. Applicants must not possess any outstanding obligations to the Department of Motor Vehicles, State of Massachusetts, or City of Springfield.
6. Applicants are required to submit a City of Springfield Taxi or Livery license application.
7. Applicants must present proof of U.S. citizenship or legal residency in the U.S.
8. Applicants must submit proof of applicant’s current legal address.
9. Applicants are required to submit their fingerprints and photo through the Springfield Police Department Photo Lab.
10. Applicants are required to submit a completed Medical Certification Form.
11. Applicants must submit a Criminal Offender Record Information (CORI) request form.
12. Applicants must not possess any prior convictions for driving under the influence.
13. Applicants must not possess any prior convictions for leaving the scene of property damage, or personal injury accidents.
14. Applicants must not possess any prior convictions for drug distribution, any crimes of violence specifically, assault and battery by means of a dangerous weapon or sexual assault cases. (Consideration may be taken for applicants who have been offense free in combination with his/her driving record, for a period of at least five (5) years.)
15. Applicants must process a closed status for any prior cases/arraignments.
16. Applicants must not be serving any portion of a Court ordered probation.
17. Applicants are required to pay a non-refundable application fee of $25.00 at the time of submitting your application, and an additional $25.00 licensing fee at the time of issue.
18. Applicants that are approved and licensed will be responsible for a license replacement fee of $20.00 for any duplicate license issued.
19. Applicants are responsible for reporting any address changes within seven (7) days to the licensing authority at the Springfield Police Department. All unreported changes will allow licensee to be subject to suspension.

Please note that approved applications will be discarded after being held for one (1) month. Applicant must re-apply after the allotted time expires.
